2011-2012 South Carolina Tuition Grant Appeals Form

2011-2012 FAFSA Question 23 - Drug Conviction
This worksheet must be completed since the student reported on the FAFSA that he/she had a conviction for possessing or selling illegal drugs or because he/she left Question 31 blank.

Dependency Override Checklist
Dependent students requesting review for unusual circumstance for independency status must submit the following documentation to the CSU Financial Aid Office for consideration.

FERPA Wavier Form
The Family Educational Rights and Privacy Act (1974) protects the privacy of student records.  Students must give written permission for Charleston Southern University to discuss financial and academic records.  

2011-2012 South Carolina Certification Form
Required for all students receiving the following South Carolina funds: LIFE, HOPE, Palmetto Fellows, or South Carolina Tuition Grant (SCTG).

Satisfactory Progress Appeal Form (SAP)
Students who fail to meet the required standards may appeal to the Office of Enrollment Services to regain their eligibility to receive financial aid.

2011-2012 Verification Worksheet
Required for students selected for verification. This form must be returned to CSU with a copy of the student's and parent's (or spouse's) signed 1040 tax forms. Important: Your financial aid cannot be disbursed until you complete the verification process.
