Immunization Form

NOTE: This exact form does not have to be used to provide the students immunization record. However, all of the requested information must be provided and signed by an official health care provider no matter what form is used.

Housing Application/Roommate Questionnaire: To complete and submit your Online Housing Application/Roommate Questionnaire, you will need to log into your MyCSU account, click on the New Student tab, and click on Housing Application/Roommate Questionnaire. Students must be accepted and have received their CSU Student ID and PIN in order to log into their MyCSU account.”
